you to the application packet for membership with
the Arbutus Volunteer Fire Department Auxiliary.
Once completed the application should be returned (A.V.F.D.A.
5200 Southwestern Blvd., Arbutus, Maryland 21227) along with $12.00
dues and other documents listed below.
Please make your check or money order payable to A.V.F.D.A. and enclose
with all other required documentation attached in order for your
application to be processed.

Certified
Copy of your driving record from the Motor Vehicle administration if
you plan to drive any AVFD vehicles.
Please ensure that it shows your correct, current address

photo copy of your drivers license

Office
of the State Fire Marshall Criminal Investigation Form.
Once
the
information above has been obtained please mail or drop off your
signed application package along with $12 cash, check or money order. Once your application has been received you will receive
the State Fire Marshall's investigation form. This
form must be filled out and submitted back to the
department before the interview with the president can be arranged.
After
your application has been processed, reviewed, and all eligibility
requirements are met, you will be contacted by the AVFDA presiding
President for a short interview.
You may be asked to appear at an interview with our Board of
Directors and/or Auxiliary Officers of this department.
